What are your new year resolutions with regards to your health

Image result for happy new year picture for 2016

Every year we make promises that we end up not fulfilling. For us to achieve all these it is better we split them into feasible units. When you decide to take everything all at once it becomes difficult to achieve.
I know top on the list of ladies will be to shed some weight if they obese. For the guys, may be to reduce the belly fat not necessarily having a 6 pack abs.
I just highlighted some of the habits we should try to keep in this coming years

  • Removing your make up after retiring for the day
  • Stop shaving off all your eye brows
  • Avoid making styles on the head that will cause traction alopecia
  • Try to change your tooth brushes regularly and visit your dentist at least twice in a year
  • Shed those fat to prevent obesity, heart diseases, hypertension etc
  • Stop eating late at night
  • Always visit your doctor when you are ill. please stop visiting pharmacies when you are ill. They are there for you to purchase drugs already prescribed by your physician.
  • Eat healthy
  • Drink in moderation
  • Do yearly check up
  • Drink lots of water
  • Quit using body creams that bleaches your skin
Nobody is perfect, I am also guilty of some of the things I mentioned above. This new year will be a better one for us. We can achieve it by doing them in bits.
I think the list is endless, please do add to the list.
I wish you all a prosperous new year.
Stay healthy guys .
Thanks for visiting .

Giant hairball weighing ONE KILO pulled from girl's stomach

Hair ball

An 11-year-old girl was rushed to hospital after she had an agonising tummy ache and couldn’t eat or even drink a glass of water any more.
Doctors decided to carry out immediate surgery but they couldn’t believe what they pulled out.
Inside the girl’s body was a humungous hairball which weighed more than a kilo – the same weight as a whole pineapple or a melon.
Doctor
The black cluster filled up 17cm of the youngster’s stomach and it continued to stretch 25cm into her bowel.
The shocking ”discovery” was made in the city of Taraz in southern Kazakhstan's Jambyl Region.
It wasn’t long before the girl’s parents got to the bottom of how the huge amount of hair mysteriously appeared in her gut.
They said that she’d often been spotted gnawing on her long dark locks but they didn’t realise that it was bad for her – or that she was swallowing the hair she chewed off.
Hairball
After the hairball, which scientists call a "trichobezoar", was removed the medics said that the girl would be able to eat and drink again.
The extremely rare condition that the girl was suffering from is known as  which causes severe stomach problems and an be fatal if it’s not caught in time.
The syndrome, in which a hairball is found in the stomach with its tail in the colon, mostly affects young women but there have been less than 120 cases of it reported in medical history.
It’s caused by trichophagia which is when a person is addicted to eating their own hair.
The human body can’t flush hair out of the digestive system so it gets stuck and matted into a big ball.
In less serious cases than this one, sufferers can be given medication to make them vomit.
Source: Daily star
I haven't seen any case in my  practise as a doctor, but I have seen cases of objects like tooth brush and coins that were swallowed accidentally.
We just have to keep an eye on our children and stop any bad habit they tend to always do and also take them to a doctor for evaluation because trichobezoars could be associated with psychiatric disorders.
Thanks for visiting and please visit again.

End of Ebola transmission in Guinea

Portrait of Raynatou Diallo, Laboratory Manager at the Télimélé District Hospital, Guinea. A town that overcame Ebola.

Geneva, 29 December 2015 - Today the World Health Organization (WHO) declares the end of Ebola virus transmission in the Republic of Guinea. Forty-two days have passed since the last person confirmed to have Ebola virus disease tested negative for the second time. Guinea now enters a 90-day period of heightened surveillance to ensure that any new cases are identified quickly before they can spread to other people.
“WHO commends the Government of Guinea and its people on the significant achievement of ending its Ebola outbreak. We must render homage to the Government and people of Guinea who, in adversity, have shown extraordinary leadership in fighting the epidemic,” says Dr Mohamed Belhocine, WHO Representative in Guinea. “WHO and its partners will continue to support Guinea during the next 90 days of heightened surveillance and in its early efforts to restart and strengthen essential health services throughout 2016.”
A milestone for the Ebola outbreak
The end of Ebola transmission in Guinea marks an important milestone in the Ebola outbreak in West Africa. The original chain of transmission started two years ago in Gueckedou, Guinea in late December 2013 and drove the outbreak which spread to neighbouring Liberia and Sierra Leone and, ultimately, by land and air travel to seven other countries.
“This is the first time that all three countries – Guinea, Liberia and Sierra Leone – have stopped the original chains of transmission that were responsible for starting this devastating outbreak two years ago,” says Dr Matshidiso Moeti, WHO Regional Director for Africa. “I commend the governments, communities and partners for their determination in confronting this epidemic to get to this milestone. As we work towards building resilient health care systems, we need to stay vigilant to ensure that we rapidly stop any new flares that may come up in 2016.”
In addition to the original chain of transmission, there have been 10 new small Ebola outbreaks (or ‘flares’) between March and November 2015. These appear to have been due to the re-emergence of a persistent virus from the survivor population.
Among the challenges survivors have faced is that after recovering from Ebola virus disease and clearing the virus from their bloodstream, the virus may persist in the semen of some male survivors for as long as 9-12 months.
WHO and its partners are working with the Governments of Liberia, Sierra Leone and Guinea to help ensure that survivors have access to medical and psychosocial care, screening for persistent virus, as well as counselling and education to help them reintegrate into family and community life, reduce stigma and minimize the risk of Ebola virus transmission.
Sustained support to Guinea, Liberia and Sierra Leone
“The coming months will be absolutely critical,” says Dr Bruce Aylward, Special Representative of the Director-General for the Ebola Response, WHO. “This is the period when the countries need to be sure that they are fully prepared to prevent, detect and respond to any new cases.
“The time-limited persistence of virus in survivors which may give rise to new Ebola flares in 2016 makes it imperative that partners continue to support these countries. WHO will maintain surveillance and outbreak response teams in the three countries through 2016.”
At the same time, 2016 will see the three most-affected countries implementing a full health sector recovery agenda to restart and strengthen key public health programmes, especially maternal and child health, while continuing to maintain the capacity to  detect, prevent and respond to any flare-up of Ebola.

Keep those items out of reach of children

This post was inspired by what happened to a paediatric patient some time ago at the hospital. The mother rushed the child to the hospital crying and a history of iron tablet ingestion was gotten. The mother was pregnant and placed her routine antenatal drugs at the reach of the child. The child ingested a lot of of the iron drugs making his health condition bad.
Children are difficult to control, this minute they are here, the other minute they are there. They could ingest harmful liquids, swallow solids that could obstruct their intestines or stuff them in the nose or ears, fight with sharp objects, suffocate themselves with plastic bags etc
This is commonly seen in children who stay in few rooms (or what we call 'face me, I face you apartments) because is difficult for items to be placed in the right places. You could see containers with kerosene, fuel, bleach placed under the beds or just by the side of the door. These children could ingest them if there are no adults with them.
There are wipes that come in a plastic container and for me I usually use them for other things once the wipes are exhausted, to store items at home and keep them away from the children. Am sure mothers will know what I am talking about. We use it to clean babies butts, face etc. I label them and stow them on the shelf or in the cabinets. There are also storage containers and jars in the market you could also use them and label them properly.

Neatly labelled storage containers

Hair beads used for the little girls

Piggy bank
Shelf

Kitchen Cabinet

You could do them the following ways

  • Stationeries :like Pencils, Erasers, Pencil sharpeners, Crayons, etc
  • Hair accessories : Hair clips, Beads etc
  • Sewing tools/materials; Needles, Pins,Hook and eye, Thimble, Buttons, Scissors, Razor blades etc
  • Nail tools/materials; Nail removers, Nail polish, Nail clippers,Nail file etc
  • Piggy bank ; Some people travel out of the country and bring back coins, you could put them there.
  • Drugs; Put them in a cabinet
  • Knives ; Put them high up in the kitchen cabinet or shelf
Please for the kerosene, fuel and bleach keep them above their heights like putting them on a shelf. This goes for any other item not mentioned here in the post that you feel is harmful to children please put them high up out of their reach.
We have carpenters in our areas that can do any of the shelf or cabinet for you at a very cheap price.
Always remember that prevention is better than cure .

Be on the alert!!!

The day was going on well not until around past the hour of 8.00 at night, when a female patient walked into the consulting room accompanied by 2 men. Her hair was looking unkempt. I just couldn't wait to start taking my history to know what to do for her.
Patient; Good evening ma
Doc; How are you? Do sit down. What is the problem?
Patient ; I just thank God for my life today
Doc ; What happened ?At that moment I paused a bit and was curious to know what happened.
Patient: I was raped by 2 armed robbers
Doc; Am so sorry . What actually happened?
Patient; I visited my cousins and on way back , I boarded a taxi to take me back home. As we got to the highway, I noticed that somebody was trying to come out of the booth into the car. He had a gun in his hand , he asked me not to shout otherwise he will kill me. They took me to a place where they collected my gold necklace, ear rings, bracelet, wrist watch, phones, the money in my wallet, luckily for me I didn't have my ATM cards in my wallet. There was a woman among the gang, she was the one that loosened my human hair from my head and collected it.
We had to run some tests for her , gave her some medications and started her on PEP .Sometimes these brutes could push you out of a moving vehicle.Patients could present with bruises , lacerations on the body or even fractures. Cases like these increase during festive periods. This happens frequently in Abuja city when you take drop.
Security tips:

  • Always board a registered taxi that the booth is not connected to the interior of the car.
  • Always ask the driver to open the booth to check if anybody is hidden in the booth
  • Always ask the driver to switch on the inner lights to check inside the car as well
  • Always check if the driver intentionally put the child lock on, so that you can easily leave  the car when you sense trouble.
  • Always direct them to take the route where it is still busy and you are familiar with. Some of them intentionally take lonely routes especially under the bridges where their accomplices are waiting to pounce on their prey. Some of of them know that people have started searching the booths and being security conscious, so they device a means to still beat that.
  • Some of them could engage you in a conversation to know the languages you speak. They call their members to let them know they have a victim to be devoured that night especially those that do not have them in the booth, so decide to take the lonely route.
  • Please always leave early especially if the route to your destination is lonely.

If you have other security tips, please don't hesitate to share with us.
Stay safe guys.
Thank you for visiting.

Patient; Yes doc
Doc ; when did this happen ?
Patient; 2 days ago

Patient: No doc
Doc; Did you smoke something?
Patient; No doc
Doc; Talk true oo make I know how I go help you oo
Patient: ok doc I took 1 wrap
Doc; Are you sure is only 1?
Patient ; I think is 2
Doc; Did you mix it with any other thing?
patient: Yes , I also had spirits that evening
Doc ; so what happened next?
Patient; I got home after that and met my wife and since then the thing no gree come down.
While this history taking was going on patient was already in A & E and the urology team invited to handle the case.
This is a case of Priapism.
What is Priapism
This can be defined as an abnormal persistent erection of the phallus. It is expected that the phallus becomes flaccid after sexual activity. For those that do not arise as a medical condition. It could be seen as a result of complication of pharmacologic agents ( recreational drugs also)
Priapism lasting for more than 4 hrs could have long term complications.
My patient waited for 48 hrs before presenting to the hospital, after all they tried at home failed to restore his phallus .
Complications like erectile dysfunction can occur leading to infertility.
Please never be ashamed of presenting to the hospital and don't start tying out what friends and neighbours are telling you if they are not doctors. The more time you waste trying out the  solutions they have given ,the more chances of you getting a poor prognosis of your disease.
Feel free to ask questions.
Thank you for visiting and do visit next time.

What do you know about premarital screening?

Marriage is a thing of joy that should be enjoyed by both partners, not a thing to endure. Family, friends, colleagues, neighbours are always happy to hear that one of their own is getting married.
In the past few days its been so disheartening for me, patients coming to the ER(emergency room) with bone pains crying and begging you to relieve them of their pains. The analgesics work for few hours and they are back to the excruciating pains. As a doctor you can't command the pains to go away, you can only commence your treatment as soon as they present and hope that they recover as soon as possible, show empathy to the patient and the relatives. Is usually not something to experience personally, trust me on this. I don't wish my enemies to have such a pain.
Premarital screening tests done in our environment includes the following:

  • HIV test
  • Genotype
  • Hepatitis B and C viruses
The HIV and Hepatitis B & C are usually not so much of a problem because we have discordant couples(where one partner is positive and the other negative),with drugs the couple can live their normal lives and also protect their children from getting infected.  I will dwell more on the genotype for this very post.
The genotype is where we always have a problem, if the couple after knowing their genotypes still decides to marry i.e AS and AS or SS and AS or SS and SS
What is sickle cell disease(SCD)?
SCD is an inherited disease of red blood cells which affects the haemoglobin. There are 3 types ;AA, AS, SS .A lot of us do not know about it but some of us know about it and  still go ahead to marry when the possibility of having a child with SCD is there.  This is where you hear things like;we love each other, he is so wealthy I want to secure my future and that of my children, he is so handsome, we have known each other for years we can't just separate like that. All these reasons are so vain compared to the agony this child will face if he or she has SCD.

The above diagram shows the outcome in each pregnancy for a couple with the trait(AS).
Nigeria has the highest burden of SCD in Africa. We can prevent it by knowing our genotypes now and make informed  decision when we are about to get married.

Both parents and the child/children suffer major social and economic consequences in the following ways:
  • A lot of money is spent during their admissions, they could stay up to a week in the hospital. Money is also spent for their routine drugs which they take through out their life.
  • You will always be away from work to take care of the child during admissions. If you work in a private sector, when is time to lay off some staff you will be no 1 on their list because you are always away from work to take care of your child and not contributing to the growth of the organization. Source of income is lost when it happens.
  • Depression: Psychologically, you will be depressed seeing your child/children in pain and yet you can't help. You can't sleep, work, all energy is on the child for him or her to recover.
  • There will be a strain on the marriage. That is the time couples begin to blame each other if they are not strong enough to go through all the stress.
  • The child could get other diseases during blood transfusion if the blood is not well screened.
  • In some cases these children tend to repeat classes because they miss exams due to their crises. Stress is a predisposing factor to their crises, they sometimes are on admission during their exams. They either defer the exams or they do not perform well due to poor preparation. This too can depress the child, seeing his or her contemporary ahead of him or her.
  • The reality is that some day the disease will take him or her away from you. The wish of every parent is for their children to take care of them when they get old.
For already affected children always try to get their routine drugs, stay away from triggers that could lead to frequent  crises, let them have their vaccines. Recently, University College Hospital (UCH) Ibadan, made it known that  bone marrow transplant is a therapy that could bring cure to SCD . This will definitely see children affected with SCD smile once again.

Feel free to ask questions and also share your experiences.
Thanks for visiting .

Learn more about 'Non-occupational PEP'

A lot of us do not know that there is a term known as 'Non-occupational PEP'. Let me define the meaning of PEP and from there we can relate it to the post .
PEP simply means post-exposure prophylaxis. It is a therapy(drugs) given to individuals who are exposed to either HIV infection or Hepatitis B infection, to reduce the risk of becoming infected. But for the scope of this post I will limit it to HIV infection.
I just want to use this platform to enlighten people about the need to visit the hospital when you run into such problems. I have had several patients who said they never knew they could access health care services in such cases.
As a medical personnel I have seen a lot of cases where patient become  positive later in life  after being exposed.
Please girls let us always be careful in our dealings with people. A patient told me of how she was raped in a man's office where she went to seal a  deal for the company that hired her. Unfortunately for the young lady the man had a different agenda, and the man  used the opportunity to unleash the animal in him on her by raping her in his office.
All she did was to take contraceptive to avoid getting pregnant but had no knowledge of coming to the hospital to get PEP if  negative for HIV.
The following people are eligible for non occupational PEP, if negative for HIV:
  • Cases of rape
  • Burst condom during sexual activities with a partner you are not aware of her status
  • Needle prick from unknown sources that you feel you are exposed to or any other case at all.
Never be ashamed of coming early  to the hospital to seek help in such cases.
Feel free to ask questions.

Stay safe guys.

My yearly birthday gifts

A lot of people celebrate their birthdays yearly and remember to put it up on facebook and other social media platforms where people send in birthday wishes to them, upload pictures of the birthday celebrations on different social media platforms where they get very nice comments. Some of us are not aware that there are some medical tests that we should do yearly just as we celebrate our birthdays.
There are medical tests that we need to carry out yearly once we attain a certain age, most especially if you are predisposed to having such a disease, if you are of a certain gender, if there is history of such diseases in the family.
I recommend we all do the following tests/examinations yearly

  • Blood glucose
  • Lipid profile
  • PSA (prostate specific antigen) in males
  • Pap smear for cervical screening in females
  • Breast Screening in females
  • BMI (Body mass index)

I have seen several cases at the ER( emergency room) of young patients brought in unconscious and when the blood sugar or blood pressure is checked is quite high and further tests could reveal kidney damages or to other organs. I recommend we check all these tests especially if you have parents or siblings with diseases like diabetes, hypertension, heart diseases, breast cancers, cervical cancers, prostate cancers etc .
This will help detect early onset of such diseases, and avoid further complications.
Blood pressure is  always a routine examination done whenever you visit the hospital and could also be checked when ever you walk into a clinic or hospital even if you are not sick.
If you are  unaware of the health status of your parents take them to a hospital so that their blood pressure ,blood sugar and other tests could be done.
Breast self examination should be done routinely by females to detect breast lumps
Always consult your doctor for these, other tests you could also do and also for further clarifications.
Let these tests be your yearly birthday gifts you give to yourself in addition to the one that will circulate the social media.
Keep in mind that prevention is better than cure.
Always stay healthy people.

Beauty gone bizarre

We always want to jump on the bandwagon to imitate both our foreign and indigenous celebrities in all they do. You have piercings done on the ear lobe, septum of the nose, on the eye lids, tongues, navel, nipples, and even on the cli---is all in the name of fashion. The first picture of a swelling on the ear shows what is called 'Keloid' In an attempt to have multiple ear piercings she developed keloid on her ear. The 2nd and 3rd pictures are pictures of multiple ear piercings and that of the septum of the nose respectively.  Africans are predisposed to developing keloids and is least seen among whites. It is commoner in females because of the multiple ear piercings and also seen more in younger people.
Please before attempting to have any piercing on any part of your body, you must find out the following;
1. Are there scars on your body  either from previous injuries or surgical incisions that have developed keloids?
2.  Are there  members of your family that have keloids on their  bodies?
It becomes very expensive and difficult to get rid of keloids once they are developed and also you are faced with complications that comes with the therapy. Remember beauty is only skin-deep.
Stay healthy guys.

Welcome to my blog

Hello guys welcome to Dr Isu-Egwu Stella's blog. I am so excited to have my own health blog. I want to use this blog to teach people about their health and to share with them the experiences I have garnered in the past years . Is really shocking to see what people still do in this 21st century to their children when they are ill  or even to others all in the name of treating them before bringing them to a health centre. I want to use this platform to enlighten us about our health and also for us to share our experiences so that others can learn from it. Once you are well informed about your health, frequency to the hospital is definitely reduced, Just like they say 'Prevention is better than cure'.
Once again you are welcome.
